The type of crown available under this offer is a Zirconia Monolithic crown only.

Conditions:

• This promotional offer is only valid for treatment commenced on or after Monday 5th of August 2024 and must be completed on or before Monday 30th of September 2024.
• The same dentist must be seen for both the impression and issue (collection) of the Zirconia Monolithic crown.
• The impression for the crown must be taken on or before Monday 30th of September 2024.
• The follow up appointment for the issue (collection) of the Zirconia Monolithic crown must be on or before Tuesday 15th of September 2024.
• If the Zirconia Monolithic crown is not issued (collected) on or before Tuesday 15th of September 2024, the promotional discount of $300 off the price of the Zirconia Monolithic crown will no longer be available and the amount payable will be the normal price as stated below.

Payment:

• A non-refundable pre-payment of $300.00 for the Zirconia Monolithic crown must be made on the day that the mould / impressions for the Zirconia Monolithic crown is made.
• The balance, including any insurance benefit is payable at the issue (collection) appointment.
• The normal price of a Zirconia Monolithic crown is specific to each patient’s needs, and in this case the normal price would depend on dentist’s assessment. If the above conditions are not met, you will not be eligible for the promotion and you will be liable to pay the normal price.
• Any existing overdue account must be settled prior to receiving any treatment, including any promotion.
